ω-3鱼油脂肪乳对急性肺损伤患者早期炎性反应和呼吸功能的影响

【摘要】目的观察ω-3鱼油脂肪乳对急性肺损伤(ALI)患者早期血清炎症介质释放和呼吸功能的影响。方法 急性肺损伤患者60例随机分为研究组和对照组,每组30例。两组患者接受急性肺损伤常规治疗同时,均接受等热量、等氮的全胃肠外肠外营养(TPN),每日所需热量为105kJ(25kcal)/kg、氮摄入量0.2g/kg,其中研究组加用ω-3鱼油脂肪乳0.2 g/(kg·d),在试验前和试验第7天凌晨抽取股动脉血和外周静脉血,检测动脉血气分析、白细胞介素-10(IL-10)、血清肿瘤坏死因子-α(TNF-α)和超敏C反应蛋白(hS-CRP)等,观察氧合指数变化和机械通气时间。结果研究组血清hS-CRP、TNF-α水平均明显降低,氧合指数改善,机械通气时间缩短。结论早期应用ω-3鱼油脂肪乳可降低急性肺损伤患者炎症反应,改善氧合,减少机械通气时间。

The effect of ω-3 fish oil emulsion on early inflammatory response and respiratory function in acute lung injury patient  
AbstractObjective: To investigate ω-3 fish oil emulsion affects inflammatory response and respiratory function in acute lung injury(ALI) patients
                         Methods:Sixty patients with ALI were randomized either to treat with fish oil emulsion 0.2g/(kg·d) (study group,n=30) or conventional therapy (control group,n=30),based on the same common treatment of ALI and total parenteral nutrition (TPN) . The artery blood gas analysis and serum concentrations of interleukin-10( IL-10)、tumor necrosis factor-α(TNF-α)、hS-CRP were measured before TPN and on the seventh day after starting TPN.The change of oxygenation index (OI) and the period of ventilation were observed.
                         Results: Patients treated with ω-3 fish oil emulsion had significantly lower in TNF-α and hS-CRP level,improved oxygenation index and decreased the period of ventilation.
                       Conclusion: TPN supplemented with ω-3 fish oil emulsion diminishes inflammatory response,improves oxygenation index, and decrease the period of ventilation.
